Enhanced Access Service

We offer appointments with a variety of clinicians six days a week, enabling patients within Dartford, Gravesham and Swanley to schedule appointments around their commitments, in the evening and at weekends.

Home Visiting Service

Home visits by a doctor can take a lot of time out of a GP’s day and may not take place until later in the afternoon. Our team of paramedics and nurses are able to see patients at home at the request of their GP if they need urgent care, are housebound or are struggling to get to the surgery.

Urgent Treatment Centre

We provide GPs that work within the local UTC. If a person’s condition is urgent enough that they cannot wait for their next GP appointment but do not need emergency treatment at A&E, they can see one of our GPs by calling 111 who will make an appointment. Alternatively, patients can simply walk into their local UTC, be triaged by a healthcare professional and wait to see one of our GPs.

Frailty Service

Surgeries have been provided with a frailty nurse who works across their PCN, who particularly focuses on Treatment Escalation Planning (TEP) in housebound patients and Care Home residents. This service works proactively with patients and residents to review their health, medication and personal circumstances to identify and act on early warning signs that might lead to a deterioration in health and wellbeing.

Enhanced Health in Care Homes

Our service provides a clear framework for delivering healthcare through the support of multi-disciplinary teams including primary care, specialists, community-based care services and care home staff. Enhanced health in care homes can be achieved by close co-ordination between care homes and the range of health services required to meet the needs of older people living in the care homes.

Physical Health Checks

We deliver the six nationally recommended physical health checks for patients. These checks cover blood pressure, BMI, blood glucose, blood lipids, smoking status, and alcohol use.

Working closely with GPs and practice teams, our service helps to identify health risks early, support patients in managing both their physical and mental health, and improve overall health outcomes. By providing dedicated clinics and targeted support, we aim to make these checks more accessible while also reducing practice workload.
